  • Please complete a new form for each student in your household, school or program. For bulk requests, please call 502-565-8397.
  • We will try to add age appropriate keepsakes and activities in your student's kit, as available.
  • We will make an effort to personalize the items included in the activity kit based on the age, gender, favorite color and interests of each student.
Not everyone will get a t-shirt, but if you happen to wear one of our giveaway sizes, we will be sure to set one aside for you!
  • Please let us know which JCPS school your student attends, or if they are in another district. We are in the process of determining which schools and districts are in greatest need. Your requests will help us make those determinations sooner.
In an effort to make as many FREE Activity Kits as possible available to students in need, we ask that families that answer "no" to this question please consider ordering a kit for your students using our fundraiser website. Your purchase helps us offset the costs so we are able to give more of them away.
For the third year in a row, Louisville PTO, Dear JCPS, 15th District PTA and others are working together to bring warm coats (and now books) to JCPS families in need. Due to the recent tornados in Western Kentucky, we desire to make this a statewide effort and we will be reaching out to impacted districts in the near future to let them know when we have coats available. There is an intake form on the Louisville PTO website that parents and FRYSCs can use to provide age, gender and other info of those in need so that our organizations can efficiently match and deliver coats and books with intention. I believe the 15th District PTA has a similar tool they use with FRYSCs, and we’ve been able to cross reference coats and needs with each other in order to serve a wider population. Highland Cleaners has offered to dry clean the donated coats and there are collection barrels in the lobbies of two of their stores. We are looking for more donation locations around Louisville. If you know of any retail or corporate establishments willing to serve as collection sites, and can let their audiences know about this effort, this would really help us provide more families in need with warmth and comfort during a brutally cold winter. Thank you.
  • Be sure to let us know anything we did not ask.
  • I am the guardian for the student named above and I give permission for you to provide them with an All History Matters activity kit at no cost.
  • In case we have trouble finding your address.
